During a critical illness or accident in your home, when you can't speak for yourself, when you are confused, in pain or scared, the Vial of L.I.F.E. can speak for you. The Vial of L.I.F.E. puts your medical information into the hands of emergency responders. This information can assist emergency personnel in administering the proper medical treatment.

Each Vial of L.I.F.E. contains:

         - Medical Information Form
The form provides a place for you to include information  about your current medication conditions, medications, emergency contacts, insurance and hospital preferences. 

          - Plastic Medication Type Vial
Place the completed medical form in the vial and then place it on the top shelf inside your refrigerator door. The vial is large enough  to hold a copy of your durable power of attorney and/or pre-hospital Do-Not-Resuscitate form.

           - Vial of L.I.F.E. Sticker
 Place the sticker on your front door or an adjacent window to alert First Responders

             - Vial of L.I.F.E. Magnet
  Place the magnet on the outside of your refrigerator to alert First Responders.

The Vial of L.I.F.E. is offered free of charge. You can obtain a vial by calling (707) 584-2641 or pick one up at the Public Safety building located at 500 City Center Drive during business hours.
